Request An AppointmentPatients may make an appointment at one of our eight locations either by calling the office or using our convenient online appointment request system. Upon submitting an online appointment request, a staff member will contact the patient to finalize the appointment time and obtain any additional information needed.
Accepted InsuranceAnkle & Foot Associates accepts most major insurance plans along with Medicare and Medicaid. If you have any questions regarding your insurance you may contact one of our offices for further information.
Patient PaperworkTo reduce wait time while checking in at one of our eight locations, patients may print and fill out paperwork prior to their office visit.
